File upload field for questionnaires

Questionnaires can now be configured with File Upload questions. This can be helpful in workflows where you'd like to send a candidate a questionnaire and allow them to upload files when submitting their questionnaire.

When a candidate submits their questionnaire and uploads a file, this will show in the Forms tab of the candidate profile. The uploaded file will be hyperlinked for quick access (in addition to the uploaded resume, if the candidate submits this on the questionnaire).
